Business and project context
Keep sensitive credentials out of the form. Collect them later through an approved access method, with the smallest permissions the work needs.
- What is the business name and the main contact for this project?
- What customer problem should this project help solve?
- Which audience and customer journey matter most right now?
- What exists today, and what must remain unchanged?
- Which services or deliverables are in scope? What is explicitly out of scope?
Ask for links to the current website and relevant public material. Do not ask a client to paste account passwords into a text box.
Content, systems, and dependencies
- Who owns the existing domain, hosting, CMS, brand assets, and analytics accounts?
- What content is current, and what needs writing, verification, or permission?
- Which forms, booking tools, payments, email systems, or other integrations are involved?
- Are there legal, privacy, accessibility, industry, or internal review requirements that affect the work?
- What assets or approvals must arrive before design, build, or launch can progress?
For each dependency, name an owner and a way to confirm completion. “We will send it later” is not a usable production plan until someone knows what “it” includes.
Decisions and communication
- Who supplies source facts? Who gives feedback? Who is the final approver?
- How should feedback be consolidated so the team does not receive conflicting direction?
- What decision must be made early, and what can remain flexible?
- What are the desired milestones, and what makes those dates realistic or uncertain?
- Who will operate and maintain the result after handoff?
What happens if the person who can approve the work is unavailable? Agree on a backup or a pause rule rather than guessing approval.
First working session
Use the completed form to confirm the outcome, scope, key risks, evidence gaps, and next action. Record the decisions in a shared project brief. Revisit them when the project changes. A form is not a contract and does not replace an approved scope or access process.
If the project includes brand work, use the branding questionnaire. For a site release, use the website launch checklist. Talk to a specialist if you want to discuss the project path with Scayver Graphix.
